Becoming a patient
Dr. Dana Mortazavi and Dr. Danae Holenka are both accepting new patients. Joining takes three steps, all online.
Before you start
Booking and your intake form are handled by the clinic’s secure booking system, not this website. Please don’t send health information by email.
Fill in the intake form
Download the new patient intake form (PDF), then complete and sign it.
Book online
Use the clinic’s online booking and choose Dr. Dana Mortazavi or Dr. Danae Holenka.
Upload your form
When booking asks if you have anything to provide, choose Yes and upload your completed form.
Where we practise
Both doctors currently see patients at Downtown Vancouver Medical Clinic, with phone and video visits available too.
